 Missing samples [Design Issues]

Hello,
If samples of a subject are not collected:
  1. for three consecutive time-points;
  2. for three random time points;
can the data of the subject be included for analysis? or, he's excluded from the study.
Does the information about missing samples be stated apriori in the protocol? What is the maximum number missing samples allowed for a subject.
