 AUC0-72 [Design Issues]

Hi All,

kindly provide your opinion.

1. In case of HVD with long half life drugs, can we go head with truncated design,

as per Amiodarone OGD guidlines showing that
For drugs demonstrating high intrasubject variability in distribution and/or clearance, AUC truncation should not be used.

2. In case of truncated study how can we handle the missing samples in statistical calculations.

Eg.
study is truncated AUC0-72.

volunteer is in-house up to 24.00 hours.
36.00, 48.00, 72.00 hours are the ambulatory samples.

if a subject not reported to facility at 48.00, 72.00 hours sampling.

how can we use those kind of subjects pK in statistics calculations.

can we include or exclude those subjects.

Personally I never understood why for the FDA truncated AUCs should not be used for HVDs/HVDPs.* Amiodarone is an example showing its applicability (all authors from the FDA).

In my studies on 200 mg amiodarone it was only borderline highly variable (CVs ~30% for AUC and Cmax). I used AUC0–72. I didn’t see high variability in clearance (CV of λz 23%). Given what is stated in the guidance, the paper given above, and my experiences I would say proceeding with AUC0–72 is justi­fied.

  • Highly variable drug: CVw ≥30% if administered as a solution. High variability is an intrinsic property of the drug (absorption/permeation, pre-systemic and/or first-pass metabolization, clearance).
    Highly variable drug product: Any of the above and high variability in formulation-related properties (liberation, influence of excipients).
