If subject had two AEs (e.g. Headache followed by nausea); How to report these two events; separately or in one form?

You have two AEs for which you have to evaluate the incidence, the severity and the duration seperately.

at the time of reporting, how they will report?
Eg. subject experiences headache followed by nausea (after 15 min), how they should recorded & reported?
