I am going to do canada submission study. what are the forms that I have to signed by the investigator?

Dear Mr.Suchit,

You need to fill the "Qualified Investigator Undertaking" of the Health Canada. This undertaking form can be downloaded.

This undertaking must be completed by the qualified investigator responsible for the conduct of the clinical trial. The completed undertaking must be retained by the clinical trial sponsor for a period of 25 years.

Please note that the Qualified Investigator Undertaking should not be submitted to Health Canada unless requested.
