Dear All, Can sombody tell me "Is it important that a clinical(principal) investigator’s name as provided in the FDA form 1572 exactly match the name on his/her medical license?" for example, if the medical license states, “Dr. V.S. Shah” while the 1572 lists the investigator as Dr. Vipul Suresh Shah? Thanks & Regards Preet -- Edit: category changed. [Helmut] |

Hi Preet, As per 'Information Sheet Guidance for Sponsors, Clinical Investigators, and IRBs" investigator's name appear on the 1572 should contain the investigator's legal name. Thanks & Regards Vasu .M |
