IMHO: management of the sponsor, a.k.a. the Man in the Armani suit. The one who was told not to use that CRO because they're not good, but who decided to go there anyway because they're cheaper. The guy who was told that the study needed 48 subjects, but who decided to go with 36 because someone else told him that should be enough, and it's cheaper. The one who was told that the formulation was not optimal, but who said there was no time (or money) to develop a better one.
