Blank signal [Bioanalytics]

Dear forum members

I'm currently validating a method for the determination of diclofenac in human plasma. So far i have three valid pre-study runs. During the last run i got a really strong signal for my analyte (LC/MS/MS method) equal to my LLOQ but no signal for the IS. I have no signal at the zero sample for the analyte and no signal at the blank solvent sample following the ULOQ. The most probable reason for that is a sample mix-up during pre-treatment. My question is whether to reject the run or somehow provide a reason for the high signal?
