Dear Mr. Dixit,

This happened to me also. In this case we have documented the same on dispensing record (form) and drug accountability log book (in remark section: Spillage of IP (T or R) while dispensing). If IP is lost the same has to be captured and if found need to be stored separately with proper label (with sub no., period, type of treatment, date, study no., sponsor, sign and date of PI etc) with the above remark on it and remarks can be given accordingly. :-)

This is my personal experience and according to our in-house SOPs.
