See here. Counter question: What for do you need CVinter (between subjects)? In sample size estimations you need CVtotal (parallel groups) or CVintra (cross-over studies - within subjects); CVinter is irrelevant in any case.

Well, cough, ANVISA… :-D

Just for completeness: EMA (2011) requires the CV calculated from residual mean squares (i.e., CVintra for cross-overs, CVtotal for parallel groups, CVWR for replicate studies).
