 Form 1572 [GxP / QC / QA]

Dear All,

Can sombody tell me
"Is it important that a clinical(principal) investigator’s name as provided in the FDA form 1572 exactly match the name on his/her medical license?"

for example, if the medical license states, “Dr. V.S. Shah” while the 1572 lists the investigator as Dr. Vipul Suresh Shah?

Hi Preet,

As per 'Information Sheet Guidance for Sponsors, Clinical Investigators, and IRBs" investigator's name appear on the 1572 should contain the investigator's legal name.
