You have two options...

   AS AN EXPERT IN THE CLINICAL INVESTIGATION OF THE DRUG FOR THE USE

   UNDER INVESTIGATION. ONE OF THE FOLLOWING IS ATTACHED.

FDA's Help Page does not give examples what 'qualification' actually means... ;-)
